Community Based Residence Facility

Our mission at Breath of Life Residence is to help our residents and families obtain the greatest degree of quality of life and the highest level of independence achievable in our home. Our small group of seven residents is centered in respect, independence and the residents’ rights. By providing high quality, competent and reliable professional staff, we at Breath of Life Residence are able to offer each resident and their families a continuity of care and independence with a commitment to their safety and respect.

Breath of Life Residence is licensed in the State of Wisconsin to serve persons with Alzheimer’s, irreversible dementia, traumatic brain injuries, advanced age, developmentally disabled and emotionally disturbed and mental illnesses.

Breath of Life Residence serves persons that meet the State of Wisconsin Class CS Semi Ambulatory and Ambulatory, but one or more who are or incapable of responding to an electronic fire alarm and exiting the facility without help or verbal/physical prompting.

The Breath of Life Residence is located in Milwaukee. There are six bedrooms; one double occupancy and five private rooms. The home is located near several bus lines and located near Midtown Mall for shopping. Midtown Medical Center is nearby to meet our residents’ medical needs. Our home is across the street from Marshall High School’s track and field for outdoor physical exercise and activities. Our Lady of Sorrow Church is located directly across the street to meet our residents’ spiritual needs.

The Breath of Life Residence will be staffed with an on-call administrator and resident assistants who are on site 24 hours a day, 7 days a week. One to two resident assistants are staffed for every seven residents each shift. Additional staff will be available to assist residents with medical appointments, social outings and personal care. We also offer support programs through the Alzheimer’s Association and specialized day programs tailored to meet the needs of each individual within our residence.

Services

Accommodations: You are entitled to the use of a private or semi-private room and the property located in the room. You are also entitled to use and enjoy common areas of the residence. You may provide your own furnishings and personal property, but Breath of Life reserves the right to limit the number and types of furnishings if they are determined to present either a potential or real safety hazard.

Daily Meals: Breath of Life provides three meals daily. Snacks are available 24 hours a day.

Utility: Breath of Life will provide heat, electricity, water service, basic television and telephone services and central air conditioning.

Housekeeping: Rooms will be cleaned once a week.

Laundry & Linen Services: Breath of Life will launder your personal items and linens once a week.

Life Enrichment Program: Breath of Life will provide planned social, educational and recreational programs.

Staffed: Twenty-four hours a day with one to two resident assistants and one on-call administrator.

Personal Service Plan: Breath of Life will provide a personal service plan. The personal service plan is designed to provide residents with a greater personal service. Breath of Life use a personal service assessment to determine the service required prior to moving in, and also periodically throughout the residency.
